Who we are
Kaizen Rehab and Performance is a physical therapy practice run by Dr. Katrina Lee, PT, DPT, CSCS, at 42 Adrian Court, Burlingame, CA 94010. Questions about this notice go to kaizenrehabandperformancept@gmail.com.
What you send through the contact form
When you use the contact form, we receive your name, email, optional phone number, the topic you pick and your message, plus the page you sent it from and how you found the site (for example a search or an ad). We use this only to reply to you and to understand which channels bring people in. It is kept in a private spreadsheet and sent to the practice inbox. Please do not include medical records or other sensitive health details in the form; we will go over those in person.
Booking
Free calls and appointments are booked through Calendly. When you book, Calendly collects the details you enter under its own privacy policy at calendly.com/privacy.
Analytics
We use Google Analytics to see how visitors find and use this site, such as which pages are viewed and whether a booking or contact button was used. It uses cookies and collects device and approximate location data. We never send your name, email, phone number or message to Google Analytics. Our website host, Framer, also records basic visit statistics.
Advertising
If we run ads on Google or Meta, we may use their measurement tags to see whether an ad led to a booking or an inquiry. These tags never receive what you type into the form. If your browser sends a Global Privacy Control signal, advertising tags are not loaded.
Your choices
You can block or delete cookies in your browser settings, install Google’s opt-out add-on at tools.google.com/dlpage/gaoptout, or turn on Global Privacy Control. To ask us to delete a message you sent, email kaizenrehabandperformancept@gmail.com.
We do not sell your information
We do not sell or rent your personal information to anyone.
